At careLOFT, we make the enrollment process easy and transparent for individuals and families seeking quality adult day services in Phoenix. Whether you’re connected through the Division of Developmental Disabilities (DDD), AHCCCS, or a private funding source, our team will guide you every step of the way.

Eligibility Requirements

Who Qualifies

  • Adults (18+) diagnosed with a developmental disability (as defined by Arizona DDD).
  • Eligible for DDD and AHCCCS services.
  • Must have authorization for Day Treatment (DTA), Group Supported Employment (GSE), or Transportation (TRA) under their Individual Support Plan (ISP).

Private Pay Options

careLOFT accepts private pay for families not yet enrolled in DDD or AHCCCS. Want to learn more?

Contact Us for a Tour

Visit our Phoenix location, meet our staff, and explore daily activities.

Complete the Intake Packet

Fill out our enrollment packet.

Coordinate with Your Support Coordinator

careLOFT will work with your DDD Support Coordinator to obtain funding and service authorization.

Schedule a Start Date

Once approved, we’ll confirm your service schedule, transportation arrangements, and welcome you to our community.

  • Current ISP / DDD Authorization
  • Medical forms (if applicable)
  • Emergency contact and caregiver information

How long does enrollment take?

Most participants can begin within 1–2 weeks after paperwork and DDD authorization are complete.

We’ll connect you with a DDD Support Coordinator and walk you through the process of applying for AHCCCS or private pay options.

Yes! Our team will confirm if your location is within our service range and coordinate daily pickups and drop-offs.
