Complete the Intake Packet

Fill out our short enrollment packet (link to PDF or online enrollment form)

Coordinate with Your Support Coordinator

careLOFT will work with your DDD Support Coordinator to obtain funding and service authorization.

Schedule a Start Date

Once approved, we’ll confirm your service schedule, transportation arrangements, and welcome you to our community.

How long does enrollment take?

Most participants can begin within 1–2 weeks after paperwork and DDD authorization are complete.
